Sertraline — sold under the brand name Zoloft — is one of the most widely prescribed antidepressants in the world. It belongs to the class of selective serotonin reuptake inhibitors (SSRIs) and is used to treat depression, anxiety, OCD, PTSD, and panic disorder.
How Long Does Sertraline Stay in Your System?
After a single dose, sertraline takes approximately 5–6 days to be fully cleared from your bloodstream. This is calculated using its elimination half-life of 26 hours — the time it takes for half the drug to leave your plasma. After 5 half-lives (roughly 130 hours), less than 3% of the original dose remains.
However, sertraline is metabolised in the liver into an active compound called desmethylsertraline, which has a much longer half-life of 60–70 hours. This metabolite remains pharmacologically active and can take up to 2 weeks to fully clear after stopping the medication.
What Affects How Long Sertraline Stays in Your System?
- Liver function — Sertraline is extensively metabolised by the liver (CYP2C19, CYP2D6). Impaired liver function significantly slows clearance.
- Age — Older adults clear sertraline more slowly; half-life can be extended in elderly patients.
- Dose — Higher doses take proportionally longer to clear. Common doses range from 25mg to 200mg daily.
- Duration of use — After long-term use, sertraline accumulates to a steady state. Full clearance after stopping long-term treatment can take longer than after a single dose.
- Other medications — Drugs that inhibit CYP2C19 or CYP2D6 (e.g. fluoxetine, omeprazole) can significantly slow sertraline metabolism.
Sertraline Discontinuation: What to Expect
Because sertraline has a moderate half-life (shorter than fluoxetine but longer than paroxetine), discontinuation symptoms are possible but typically milder than with shorter-acting SSRIs. Symptoms such as dizziness, nausea, and "brain zaps" may appear 2–4 days after stopping and generally resolve within 1–2 weeks as the drug clears.
Never stop sertraline abruptly without consulting your doctor. A gradual taper is usually recommended to minimise discontinuation effects.
