Fluoxetine — best known by its brand name Prozac — is one of the most widely prescribed antidepressants in the world. It is a selective serotonin reuptake inhibitor (SSRI) used to treat depression, OCD, panic disorder, bulimia nervosa, and bipolar depression. What makes fluoxetine pharmacologically unique is its extraordinarily long half-life compared to all other SSRIs.
How Long Does Fluoxetine Stay in Your System?
Fluoxetine has a half-life of 1–4 days (typically around 2 days for the parent drug). After stopping fluoxetine, the parent drug itself takes approximately 5–20 days to clear from the bloodstream. However, fluoxetine is metabolised in the liver into an active compound called norfluoxetine, which has a much longer half-life of 7–15 days.
Because norfluoxetine is pharmacologically active and accumulates during treatment, full clearance of all active compounds after stopping fluoxetine typically takes 4–6 weeks. This is substantially longer than any other commonly prescribed SSRI.

What Affects How Long Fluoxetine Stays in Your System?
- Liver function — Fluoxetine is extensively metabolised by CYP2D6 and CYP2C9 enzymes. Impaired liver function significantly prolongs both fluoxetine and norfluoxetine half-lives.
- CYP2D6 genetics — Poor metabolisers of CYP2D6 (around 7–10% of the population) clear fluoxetine much more slowly, leading to higher plasma levels and longer persistence.
- Age — Older adults clear fluoxetine more slowly. Half-life can be significantly extended in elderly patients.
- Duration of treatment — After long-term use, both fluoxetine and norfluoxetine accumulate to a steady state. The 4–6 week clearance window applies primarily after sustained treatment, not necessarily a single dose.
- Drug interactions — Fluoxetine is itself a potent inhibitor of CYP2D6, meaning it can slow the metabolism of other medications taken concurrently.
Why Does Fluoxetine Require Less Tapering Than Other SSRIs?
Because of its extraordinarily long half-life, fluoxetine essentially tapers itself. When you stop taking it abruptly, plasma levels decline so slowly that the brain has time to adapt — greatly reducing the discontinuation symptoms (dizziness, nausea, "brain zaps") that commonly occur with shorter-acting SSRIs like paroxetine or venlafaxine.
This self-tapering property makes fluoxetine the preferred choice for switching patients off other antidepressants — a brief course of fluoxetine is sometimes prescribed specifically to ease discontinuation of a shorter-acting SSRI.
Fluoxetine is unusual among SSRIs because of its exceptionally long half-life: the parent compound has a half-life of roughly 1 to 3 days after a single dose (extending to 4–6 days with chronic dosing), and its active metabolite norfluoxetine has a half-life of about 9 to 16 days. Because of this slow elimination, fluoxetine generally produces fewer and milder discontinuation symptoms than shorter-acting SSRIs, and a once-weekly 90 mg delayed-release formulation is FDA-approved for maintenance treatment of major depressive disorder.
