Levothyroxine — sold as Synthroid, Euthyrox, Levoxyl, and Tirosint — is a synthetic form of the thyroid hormone thyroxine (T4). It is one of the most prescribed medications in the world, used to treat hypothyroidism (underactive thyroid), as well as thyroid cancer and goitre. It is taken once daily, ideally on an empty stomach 30–60 minutes before breakfast. Levothyroxine has one of the longest half-lives of any commonly prescribed medication.
How Long Does Levothyroxine Stay in Your System?
Levothyroxine has a plasma half-life of approximately 7 days (168 hours) in patients with normal thyroid function (euthyroid). In hypothyroid patients — who naturally clear thyroid hormone more slowly — the half-life can extend to 9–10 days. In hyperthyroid patients it is shorter, around 3–4 days.
Because of this very long half-life, it takes approximately 4–6 weeks (5 half-lives) for levothyroxine to fully clear from the body after stopping, and equally, 4–6 weeks to reach a new steady state after starting or changing a dose. This is why thyroid function tests (TSH, free T4) should always be rechecked 4–6 weeks after any dose adjustment — testing sooner will not reflect the true effect of the new dose.
Why Does Levothyroxine Take So Long to Work?
The slow onset and offset of levothyroxine is a direct consequence of its 7-day half-life. When you start or adjust levothyroxine, it takes approximately 5 half-lives — around 35 days — for blood levels to stabilise at the new steady state. During this time, symptoms may only partially improve, and TSH levels are still in flux.
Additionally, the biological effects of thyroid hormone — changes in metabolic rate, heart rate, and body temperature regulation — are mediated through gene transcription in cells, a process that itself takes days to weeks to fully manifest. This means the clinical response may lag even behind the blood level stabilisation.
What Affects How Long Levothyroxine Stays in Your System?
- Thyroid status — Hypothyroid patients clear levothyroxine more slowly (half-life up to 9–10 days); hyperthyroid patients more quickly (~3–4 days). This is because thyroid hormone accelerates its own metabolism.
- Age — Elderly patients tend to have a longer half-life and lower dose requirements. Starting doses should be conservative in older adults, especially those with cardiac risk.
- Body weight — Levothyroxine dosing is weight-based (typically 1.6 mcg/kg/day for full replacement). Higher body weight generally requires a higher dose.
- Absorption-affecting factors — Several factors significantly impair levothyroxine absorption from the gut: calcium carbonate, iron supplements, antacids, proton pump inhibitors (like omeprazole), coffee, and high-fibre foods all reduce absorption if taken simultaneously. Levothyroxine should be taken on an empty stomach, separated from these by at least 4 hours.
- Pregnancy — Thyroid hormone requirements increase significantly in pregnancy (often by 25–50%). Doses should be adjusted early in the first trimester and monitored closely.
- Malabsorption conditions — Coeliac disease, inflammatory bowel disease, and post-bariatric surgery can reduce levothyroxine absorption, requiring higher doses or alternative formulations (Tirosint gel caps).
What Happens If I Miss a Dose of Levothyroxine?
Because levothyroxine has a half-life of 7 days, missing a single daily dose causes only a minimal change in overall thyroid hormone levels — approximately a 14% reduction from steady state, which is clinically insignificant for most patients.
If you miss a dose, take it as soon as you remember. If you remember the next day, you can safely take two doses on the same day — this is explicitly supported by prescribing guidelines because of levothyroxine's wide therapeutic index and long half-life. Do not skip the missed dose entirely, as consistent daily dosing is important for stable thyroid hormone levels over time.
