Gabapentin — sold under the brand names Neurontin and Gralise — is an anticonvulsant medication originally developed for epilepsy. It is now most commonly prescribed for neuropathic pain (nerve pain), postherpetic neuralgia, and as an adjunctive treatment for partial seizures. It is also used off-label for anxiety, restless leg syndrome, and alcohol withdrawal.
How Long Does Gabapentin Stay in Your System?
Gabapentin has a plasma half-life of approximately 5–7 hours, with an average of around 6 hours in adults with normal kidney function. After a single dose, it is largely cleared from your bloodstream within 25–35 hours (5 half-lives).
A notable pharmacokinetic feature of gabapentin is that its absorption is dose-dependent and non-linear. Unlike most drugs, higher doses do not produce proportionally higher blood levels — this is because gabapentin uses a saturable transporter (LAT1) for intestinal absorption, meaning absorption efficiency decreases at higher doses. This is why gabapentin is often dosed three times daily rather than once daily.
What Affects How Long Gabapentin Stays in Your System?
- Kidney function — Gabapentin is excreted exclusively unchanged through the kidneys. Impaired renal function dramatically prolongs clearance. In severe renal impairment, the half-life can extend to over 50 hours, requiring significant dose reductions.
- Age — Elderly patients often have reduced kidney function, leading to slower gabapentin clearance and higher plasma levels at standard doses.
- Dose and frequency — At higher doses, gabapentin absorption becomes less efficient (saturable transport). Extended-release formulations (Gralise, Horizant) produce different absorption profiles than immediate-release Neurontin.
- Antacids — Aluminium- or magnesium-containing antacids reduce gabapentin absorption by up to 24% when taken simultaneously. Gabapentin should be taken at least 2 hours after antacids.
Does Gabapentin Show Up on a Drug Test?
Gabapentin does not appear on standard drug screening panels, which typically test for opioids, benzodiazepines, cannabinoids, amphetamines, and cocaine. A specific gabapentin immunoassay or confirmatory GC-MS test is required to detect it.
However, gabapentin testing is becoming increasingly common — particularly in pain management clinics, probation and parole programmes, and some employers. It is not undetectable if specifically sought. Urine detection windows are typically 1–3 days after the last dose in people with normal kidney function.
Gabapentin is absorbed from the small intestine via saturable L-amino-acid transporters, which gives it the unusual property of non-linear, dose-dependent bioavailability: oral bioavailability is roughly 60% at 900 mg/day but falls to about 27–35% at 3,600 mg/day, as reported in the FDA prescribing information for Neurontin.
